Alumni Award Nomination Form

Instructions: Each campus has separate deadlines for its awards. If you have questions regarding the timetable, please contact your alumni relations manager. Your nomination will be referred to the appropriate alumni association committee, which may contact you or the nominee for additional information. Nominations are accepted year-round. You may nominate any IADT graduate, including yourself, and current faculty with at least two years of service.

Distinguished Alumni Award:Presented to a graduate in recognition of his or her personal and professional achievements and how they reflect the mission and values of IADT.

Faculty Recognition Award:The Alumni Association will recognize a faculty member who has served the school for at least two years and has achieved personal/professional accomplishments and service to students that reflect the ideals of IADT. Recipients will reflect the values of service, leadership, and a strong commitment to student welfare.

Alumni Volunteer Spirit Award:This award recognizes a member of the Alumni Association who as an exemplary and devoted volunteer has supported the mission and values of the association and the school's endeavors.

Did he/she get promoted or continue his/her education? Does he/she participate in community service, and if so, in what way?
